This form is for providers of substance use treatment and recovery support, allied health, social care, criminal justice and housing professionals who work with people who use drugs and alcohol. It can also be used by people who use drugs and their families.

Our aim

Gathering accurate and timely information on drug related fatalities (including alcohol) is helpful for the early identification of emerging trends and putting proactive prevention measures in place. Informing the drug-related death review process also helps improve the way our services are run.

This form can be used to report the following fatalities:

  • from an illicit drug overdose
  • from a prescription drug overdose
  • from ill health caused by substance use or the associated lifestyle
  • of someone known to local substance use treatment and support services (past or present) regardless of whether the cause of death is known
  • as a direct consequence of alcohol use, for example:
    • alcoholic liver disease
    • mental and behavioural disorders due to the use of alcohol
    • alcohol poisoning.
