You need to fill in this form when you get a laptop or data SIM though the Bristol City Council digital inclusion scheme.
This form is not for requesting a laptop through the scheme or finding out if you're eligible. We'll contact you if you're eligible.
Before you get the laptop, we'll have told you:
- when the laptop will be ready
- if you need to collect or if we'll deliver it
You must fill in the form before you we can give you the laptop or SIM.
The person giving you the laptop can help you fill in the form.
